chore: 添加 .gitignore 并移除敏感/缓存文件

- 添加 .gitignore (Python __pycache__, .env, .idea, node_modules 等)
- 从追踪中移除 backend/.env, frontend/.env (密钥文件)
- 从追踪中移除 __pycache__/ 目录 (编译缓存)
- 从追踪中移除 .idea/ (IDE 配置)
